Duties
- Assess client needs and develop individualized service plans
- Connect clients with community resources, support programs, and social services
- Monitor and evaluate client progress, adjusting plans as needed
- Advocate for clients in accessing housing, healthcare, employment, and other essential services
- Maintain accurate case records and ensure compliance with regulations
- Collaborate with social workers, healthcare providers, and other professionals
- Provide crisis intervention and support when necessary
Experience
- Bachelor’s degree required
- Previous experience in case management, social services, or a related role preferred
- Strong interpersonal and problem-solving skills
- Knowledge of community resources and social service programs
- Ability to manage multiple cases and work independently
- Proficiency in documentation and case management software is a plus

Pay: $45.00 - $60.00 per hour
