What are the responsibilities and job description for the Hospice Clinical Manager position at Symbii Home Health and Hospice?
POSITION SUMMARY: Collaborate with and support the DON and Supportive Services Manager to ensure the efficient delivery of hospice services while assisting with the overall planning, organizing, direction, coordination, staff development and education, medication reviews and supervision of all nursing and professional services. Assist in the overall operation of the hospice division in accordance with guidelines and regulations that govern the organization and ensure the highest degree of quality care is maintained at all times. Act as the DON in his/her absence.
JOB FUNCTIONS:
- Assist with the determination to accept or deny an individual for admission to hospice services
- Identify educational needs of patient/clients and their families and ensure patients/clients and their family are timely and adequately educated at the time of admission and throughout their time with hospice
- Exemplify and promote positive, professional and effective communication between and among disciplines
- Ensure case conferences are conducted as needed
- Consult with and address nursing concerns and practice
- Ensure clinical records are audited to identify non-compliance and to identify opportunities for instruction
- Ensure on-site supervisory visits are conducted for clinical staff at least one (1) time per calendar year to identify opportunities for instruction
- Develop and maintain professional relations with members of the health care delivery system, including but not limited to physicians, nurses, discharge planners, building administrators
- Maintain an adequate knowledge of regulatory requirements of the hospice industry
- Maintain an adequate knowledge of actual and potential changes to the hospice delivery system
- Maintain an understanding of agency operations
- Collaborate with the DON and administrator to review hospice policies and procedures
- Collaborate with the DON and Supportive Services Manager to identify and recommend alternative methods for service delivery
- Participate in weekly IDT meetings
- Participate in the orientation of new employees
- Keep the DON informed on all events which occur within the agency
- Advise the DON as to basis of contracting health services to be provided
- Develop, evaluate, review, revise and implement policies and procedures; goals and objectives; and standards of care relevant to patient services and staff functions
- Perform patient care responsibilities considering needs specific to the standard of care for patient’s age
- Maintain and ensure patient confidentiality at all times
- Report to work on time and as scheduled
- Attend annual review and perform departmental inservices
- Represent the agency in a positive and professional manner
- Ensure compliance with policies and procedures regarding Hospice operations, fire safety, emergency management and infection prevention and control
- Effectively and consistently communicate administrative directive to staff and encourage interactive departmental meetings and discussions
- Comply with all organizational policies regarding ethical business practices
- Communicate the mission, ethics and goals of the Hospice agency
POSITION QUALIFICATIONS:
- Must possess nursing degree from an accredited school in social work or related field
- Must be a licensed nurse in the State of Wisconsin
- Minimum of three years of experience in hospice care
- Current knowledge of hospice regulations
- Valid state driver’s license and reliable transportation
- Proof of current automobile insurance

Pay: $85,000.00 - $93,000.00 per year
