Overview
Symbria Rx Services is seeking a PRN Consultant Pharmacist to add to our growing team at our Pharmacy located in Meadville, PA!
POSITION SUMMARY
The primary purpose of this position is to provide pharmacist consulting services to long term care communities by providing education and audits to ensure the nursing community is meeting regulatory requirements. Functions as a liaison between nursing community and pharmacy operations.
Responsibilities
- Drug Regimen Review (DRR) which is comprehensive monthly review of resident’s medical chart or electronic record. Including reviews of medication utilization, lab results, medical conditions, history and physical, vital signs and progress notes
- When requested, incorporate additional community requests such as fall reviews, antibiotic usage or specific medication class reviews into DRR process
- Meet upon entry and exit (if possible) with Director of Nursing and/or Assistant Director of Nursing.
- Meet upon entry (if possible) with Administrator and/or Assistant Administrator.
- Make recommendations to the appropriate members of the Inter-Disciplinary Team to:
- Ensure and enhance the quality of care for the resident in the consultant’s professional opinion.
- Provide guidance toward adherence of laws, rules, guidelines, and policy and procedures from a pharmacy perspective.
- Provide a monthly psychotropic class report for community use in monitoring psychotropic medication usage and attempts at dose reductions.
- Upon completion of DRR, consultant report is generated with recommendations for physician and nursing review along with summary and current month response rate to previous recommendations. Consultant report should be left with the Director of Nursing or their designee at the community.
- Attend quarterly Quality Assurance (QA) meetings and provide QA reports:
- QA report must minimally contain the following items:
- Changes to emergency and/or contingency supply.
- Antipsychotic chart and graph.
- Antidepressant chart and graph.
- Anxiolytic chart and graph.
- Hypnotic chart and graph.
- Comparison with OSCAR numbers and graph if possible.
- Summary of DRR process for the quarter, including number of medical records reviewed, breakdown of recommendations for the quarter and community response rate to pharmacy recommendations.
- Address any other pharmacy related concerns the community or physicians may raise.
- Physician recommendations:
- Deliver within three business days to Director of Nursing or designee.
- Summary report of recommendations delivered to Director of Nursing, Administrator and any other person requesting within three business days.
- Place a copy of the report on company server in the designated folder monthly for each facility visited. Update and maintain consultant software monthly.QA review of consultant activity:
- Review of charts.
- Review of recommendations.
- Paper reviews from computer printout.
- Maintain awareness of current laws, regulations, and guidelines for long term care communities, both federal and state (where applicable and licensed).
- Upon request provide in-service medication related education to nursing community.
- Ensure consulting software program is current and up to date. This includes installing monthly updates and periodic software version updates.
- File backup copies of all monthly consultant DRR reports, Quality Assurance reports and community database on company share drive for accessibility.
- Maintain awareness of Symbria Rx Services policy and procedures.
- Maintain awareness of developing trends and treatments of all aspects of long term care including, but not limited to; psychotropic drug reductions, psychotropic monitoring, appropriate drug use, and clinical monitoring.
- Any and all duties as assigned.
Qualifications
To perform this job successfully, the ability to perform each essential duty satisfactorily is necessary and the qualifications listed below are representative of the knowledge, skill, and/or ability required:
- Bachelors of Science degree in Pharmacy from NABP accredited College of Pharmacy or Pharm D degree.
- Valid state registered pharmacist license, multiple state licensure may be required.
- Long term care pharmacy (geriatric) experience preferred.
- General computer knowledge including Microsoft Office: Word, Excel and Outlook.
- Exercise professional phone, verbal and written communication skills.
- Ability to maintain organization, prioritize and multi-task. Exhibit initiative and reliability.
- Knowledge of current State and Federal Long Term Care regulations and guidelines and familiarity with the nursing home survey process.
- Must have a valid driver’s license and vehicle with valid insurance.
- Must be willing to travel up to 3 hours each way to business sites.
