Responsibilities
- Take on the role of nurse care manager for a caseload of patients and families, covering all aspects of the nursing process such as planning, executing, assessing, and evaluating.
- Review the caseload on a regular basis by communicating with the "Clinical Director."
- Ensure that patient records meet HIPAA requirements and are kept up to date in order to precisely and clearly articulate issues, plans, actions, and objectives, and to reflect any changes as they occur.
- Participate in patient conferences and interdisciplinary team meetings to encourage discussion of caseload-related concerns for staff review, consultation, and discussion.
- Takes on the duty of informing patients and their families about the on-call coverage when needed.
- Ensures that the right protocols are followed at the time of death by providing the necessary care and support.
- Takes part in professional staff orientation and training sessions offered by SympathyCare Hospice.
- Exhibits an understanding of program principles and processes as well as Hospice rules and regulations.
Qualifications
-Graduated of an accredited school of nursing and current state Registered Nurse license.
-Minimum 3 years nursing experience and 1 year Hospice work experience.
-Current BCLS certificate upon hire and maintain current.
-Knowledge and experience in basic to intermediate computer skills.
Job Type: Full-time
Pay: $36.00 - $40.00 per hour
