The MDS Coordinator will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments for our patients. Responsibilities :
- Coordinate and complete MDS assessments for patients
- Ensure accuracy and completeness of MDS assessments
- Collaborate with interdisciplinary team to develop care plans for patients
- Monitor and track patient progress and update care plans as needed
- Ensure compliance with all state and federal regulations related to MDS assessments
