Regional Clinical/Hospital Liaison

Symphony Maple Crest and Symphony Northwoods are growing their team and is hiring for a Regional Clinical/ Hospital Liaison.

Regional Clinical/Hospital Liaison Responsibilities:

• Acts as the liaison between the hospitals and the two centers by conducting hospital on-sites for clinical review and to support quality transitions of care.
• Meet with hospitalized patients and families at the bedside to answer questions.
• Work closely with the Admissions Coordinator and the centers on patient clinical and equipment needs/updates, status on transfer/admission ensuring the facility is prepared for the admission.
• If the Admissions Coordinator is out of the office, call prospective families to welcome them, offer a tour, and help understand how the transition to our care setting works.
• Create and implement marketing events with the center team to encourage referral sources and the community to visit the centers positioning them as “the place to be”.
• This position is responsible for producing growth in total referrals and admissions for both centers including hospital account integration.
• Support customer service and first impressions initiatives.
• Support positive social media postings, Google reviews, and overall on-line presence.

Regional Clinical/Hospital Liaison Qualifications:

• Minimum of two years marketing and/or liaison experience in Senior Living, SNF and LTC. Nurses are welcome to apply.
• Must have the ability to establish good working relationships with health care professionals.
• Seeking a self-directed professional with initiative who requires minimal supervision.
• Excellent observation, verbal, and written communication skills.
