Job Title: Primary Care Nurse Practitioner

Job Summary: The Primary Care Nurse Practitioner is a licensed healthcare professional responsible for providing comprehensive primary care services to patients of all ages. This role involves assessing, diagnosing, and treating acute and chronic medical conditions, as well as promoting preventive care and health maintenance.

Responsibilities:

• Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests.
• Formulate differential diagnoses based on clinical findings.
• Order and interpret diagnostic tests as needed.
• Develop and implement individualized treatment plans in collaboration with patients and other healthcare professionals.
• Prescribe medications, therapies, and other appropriate interventions.
• Educate patients on treatment plans, medication usage, and self-care.
• Manage and monitor patients with chronic medical conditions, such as diabetes, hypertension, and respiratory diseases.
• Coordinate care with specialists as necessary.
• Provide preventive healthcare services, including vaccinations, health screenings, and health education.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
• Document patient encounters, assessments, and interventions in electronic health records (EHR).

Qualifications:

• Master's or Doctoral degree in Nursing with a focus on Family or Adult-Gerontology Primary Care.
• Minimum 4 years of experience required.
• Current state NP licensure and national certification as a Nurse Practitioner.
• Previous experience in primary care is often preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a team-based healthcare environment.

Job Type: Full-time

Pay: $55.00 - $64.00 per hour

Expected hours: 40 per week
