We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) Care Manager to join our team in Pasadena, CA. This is a full-time position ideal for individuals who are passionate about providing quality care and ensuring that patients receive the support they need to achieve optimal health outcomes. The LVN Care Manager will work closely with patients, families, and healthcare teams to develop personalized care plans and oversee their implementation.

Qualifications:

• Active LVN license in California.
• 2 years of experience in nursing, with a focus on care management or home health.
• Strong patient assessment and care planning skills.
• Excellent communication and interpersonal skills.
• Ability to manage multiple cases and prioritize tasks.
• Proficient in EHR systems.
• Knowledge of healthcare regulations and HIPAA compliance.
• Ability to work well in a team environment and advocate for patients.
• Reliable transportation and willingness to commute within the Pasadena area.

Key Responsibilities:

• Coordinate patient care and ensure needs are met efficiently.
• Collaborate with healthcare teams to create and update individualized care plans.
• Conduct health assessments and monitor patient progress.
• Educate patients and families on care options, medications, and preventive measures.
• Provide direct care, including medication administration and routine assessments.
• Ensure care plans comply with regulations and best practices.
• Monitor patient adherence and follow up on necessary services.
• Advocate for patients and assist with scheduling appointments and referrals.
• Maintain accurate patient records and provide ongoing support to patients and families.

Job Type: Full-time

Pay: $40.00 - $50.00 per hour

Experience:

• Nursing: 2 years (Preferred)

License/Certification:

• LVN License (Preferred)
