Key Responsibilities:
- Care Coordination: Work closely with clients, families, and caregivers to create and manage individualized care plans.
- Customer Service: Provide exceptional customer service to clients, addressing their concerns and ensuring satisfaction.
- Administrative Support: Coordinates office management tasks, scheduling, and maintaining accurate records using AxisCare.
- Management/HR: Oversee caregiver assignments, assist in HR tasks such as hiring and onboarding, and provide ongoing support to the team.
- Bookkeeping: Manage financial records, assist with invoicing, billing, and payroll.
- Sales and Marketing: Help grow our business by participating in marketing initiatives and engaging with potential clients and partners.
- Case Management: Coordinate care and services for multiple clients, ensuring they receive timely and effective support.
- Organizational Skills: Maintain an organized workflow to ensure smooth day-to-day operations and adherence to care plans.
- Effective Communication: Serve as the point of contact between clients, families, caregivers, and healthcare providers to ensure clear, empathetic communication.
Qualifications:
- Minimum of a LPN/LVN certification.
- Minimum of 2 years of experience in a care coordination or case management role.
- Proficiency in using AxisCare or similar home care management software for scheduling.
- Excellent organizational skills, with the ability to multitask and prioritize in a fast-paced environment.
- Strong communication skills, both verbal and written, with a compassionate approach.
- Positive and gentle communication style.
- Able to handle stressful situations with calm and professionalism.
- Bilingual (English/Spanish preferred).
