We are seeking a dedicated Care Coordinator to join our compassionate team. As a Care Coordinator, you will play a crucial role in ensuring that our clients receive the best possible care while supporting our team with administrative and managerial duties.

Key Responsibilities:

  • Care Coordination: Work closely with clients, families, and caregivers to create and manage individualized care plans.
  • Customer Service: Provide exceptional customer service to clients, addressing their concerns and ensuring satisfaction.
  • Administrative Support: Coordinates office management tasks, scheduling, and maintaining accurate records using AxisCare.
  • Management/HR: Oversee caregiver assignments, assist in HR tasks such as hiring and onboarding, and provide ongoing support to the team.
  • Bookkeeping: Manage financial records, assist with invoicing, billing, and payroll.
  • Sales and Marketing: Help grow our business by participating in marketing initiatives and engaging with potential clients and partners.
  • Case Management: Coordinate care and services for multiple clients, ensuring they receive timely and effective support.
  • Organizational Skills: Maintain an organized workflow to ensure smooth day-to-day operations and adherence to care plans.
  • Effective Communication: Serve as the point of contact between clients, families, caregivers, and healthcare providers to ensure clear, empathetic communication.

Qualifications:

  • Minimum of a LPN/LVN certification.
  • Minimum of 2 years of experience in a care coordination or case management role.
  • Proficiency in using AxisCare or similar home care management software for scheduling.
  • Excellent organizational skills, with the ability to multitask and prioritize in a fast-paced environment.
  • Strong communication skills, both verbal and written, with a compassionate approach.
  • Positive and gentle communication style.
  • Able to handle stressful situations with calm and professionalism.
  • Bilingual (English/Spanish preferred).
