Overview
We are seeking a culturally aware and relationship-driven Community Partnerships Clinic Liaison to support community outreach and patient enrollment. The ideal candidate will be skilled at building trust with closely connected, family-centered communities and knowledgeable about the healthcare provider landscape. This individual will serve as a bridge between local clinics, community-based organizations (CBOs), and families, educating them on the value of home care services offered by SYNERGY HomeCare.

What We’re Looking For
Education:
High School Diploma or GED required
Associate’s Degree in Marketing, Business, Healthcare Administration, or a related field preferred
Experience:
Minimum of 3 years in a similar role within the home care or health services industry
Experience engaging with culturally diverse communities is essential
Bilingual skills strongly preferred
Proven ability to speak confidently in front of large groups
Strong organizational, written, and interpersonal communication skills
Existing relationships with local referral networks highly desired
Other Requirements:
Valid driver's license and reliable transportation preferred
Ability to travel locally and work flexible hours as needed
Self-starter with minimal supervision required
