Rate: $55 - $100 per in person visit (depending on type/purpose of visit)
Responsibilities and Duties:
The Hospice Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes, and include the following duties:
- Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care.
- Implement current nursing practice following a patient assessment and an approved plan of care initiated by the registered nurse.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
- Participate in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members.
- Use equipment and supplies effectively and efficiently.
- Provide appropriate pain/symptom management. Evaluate patient’s response to treatments/medications.
- Participate in personal, professional growth and development. Also participate in the organization's quality management program.
- Perform other duties as assigned by the registered nurse.

Position Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- At least 3 years nursing experience as LPN/LVN. Community health/hospice or medical/surgical experience is preferred.
- Currently licensed as a LVN in California
- Possess and maintain current CPR certification
- Proficiency with HospiceMD
Schedule: As Needed
Pay: $55.00 - $100.00 per day
