Hourly Rate: $40 - $45 depending on experience
We are seeking a compassionate and detail-oriented Licensed Vocational Nurse (LVN) to join our hospice team in an in-office role. The Hospice LVN will provide administrative and clinical support to ensure seamless coordination of patient care, maintain compliance with regulations, and support field staff. This position is ideal for an LVN with hospice experience who prefers an office-based setting and enjoys working in a team-oriented environment.
Responsibilities and Duties:
Clinical Coordination & Patient Support
- Assist in coordinating hospice services, including admissions, recertifications, and discharges.
- Communicate with patients, families, and caregivers to provide education, guidance, and support.
- Document every patient/family interaction in the communication note (comm note) within the EMR, including any updates, interventions, or coordination efforts.
- Input and track MD orders as needed, ensuring proper documentation and communication.
- Collaborate with interdisciplinary team (IDG) members, including RNs, physicians, and social workers, to ensure continuity of care.
- Review physician orders and ensure timely processing and implementation.
- Assist in triaging patient calls and provide appropriate recommendations based on symptoms and hospice protocols.
Quality Assurance (QA) of Submitted Notes
- Perform weekly QA of submitted clinical notes, ensuring completion prior to Friday at 11 AM for payroll processing.
- Verify that submitted notes align with the calendar/frequency of visits to ensure compliance with the plan of care (e.g., RN visit every 14 days, HA visits 2x per week, etc.).
- Confirm that notes are correctly labeled and categorized, including: (1) Recertification visits properly marked, (2) PRN visits labeled as "unscheduled", and (3) any other visit types appropriately classified.
- Communicate with staff regarding completion of outstanding notes to ensure compliance and timely documentation submission.
Medication & Physician Orders Managment
- Process and track medication orders upon request, ensuring proper documentation and compliance.
- Verify that MD orders are received and correctly documented in the system before implementing changes.
- Communicate with pharmacies, DME providers, and suppliers to facilitate timely delivery of medications and medical equipment.
Administrative & Documentation Support
- Maintain accurate and timely documentation in the electronic medical record (EMR) system.
- Track and follow up on physician orders, medication refills, and DME (Durable Medical Equipment) requests.
- Ensure compliance with federal, state, and organizational policies regarding hospice care.
- Assist with scheduling and coordinating visits for field staff.
- Support audits, chart reviews, and quality improvement initiatives as needed.
Communication & Team Collaboration
- Serve as a liaison between patients, families, field nurses, and physicians.
- Provide updates to the hospice team regarding changes in patient conditions or care needs.
- Communicate effectively with the Interdisciplinary Group (IDG) team, ensuring all team members are informed of patient updates and care changes.
- Assist in onboarding and training new hospice staff regarding documentation and procedures.
Support for Director of Patient Care Services (DPCS)
- Provide administrative and clinical support to the DPCS in all tasks necessary for hospice program success.
- Assist with compliance monitoring, regulatory audits, and operational improvement initiatives.
- Ensure seamless communication between the office team and field clinicians to uphold high-quality patient care.
Position Qualifications
- Current and valid LVN license in the state of CA.
- Minimum 1-2 years nursing experience as a LPN/LVN; hospice and/or palliative care experience is preferred.
- Complies with accepted professional standards and practice.
- Knowledge of hospice regulations, Medicare guidelines, and EMR documentation.
- Strong organizational and multitasking skills with attention to detail.
- Excellent communication and interpersonal skills.
- Proficiency in HospiceMD and Microsoft Office Suite.
- Graduate of an accredited practical nurse or vocational nursing program.
Work Schedule & Benefits
