Job Summary:
We are seeking a dedicated and compassionate Nurse Practitioner to join our team. With Florida Rehab Specialists, you will work closely with individuals with varying acute and chronic health concerns, providing support and assistance to help them achieve their goals and improve their quality of life.
Duties:
- Conduct assessments to determine the needs and goals of individuals with developmental disabilities
- Develop and implement personalized treatment plans based on individual needs
- Provide direct care and support to individuals, including assistance with daily living activities
- Collaborate with a multidisciplinary team to coordinate and monitor the progress of individuals
- Assist with discharge planning and transitioning individuals to appropriate community resources
- Administer medications and treatments as prescribed by healthcare professionals
- Collect and process specimens for laboratory testing
- Maintain accurate and up-to-date documentation of all services provided
