Masters in Social Worker (MSW)

Looking for a Social Worker for T.O.N.E. Hospice and Palliative Care, Inc. - EITHER PT OR PRN!!

What You Will Do:

• Assess hospice patients to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.

Transportation:

Must have a current valid driver’s license, auto liability insurance and reliable transportation.

QUALIFICATIONS

• Bachelors or Master’s Degree in Social Work from a State accredited program
• Current State license required in accordance with State regulations
• Valid driver’s license and car insurance
• At least 2  years of experience as a Social Worker in a clinical care setting required, hospice experience preferred
