What are the responsibilities and job description for the Monitor position at T & T Tutor World, Inc.?
Monitor Providers
- Monitor providers on your caseload 60-90 Min or more if needed
- Mail monitoring report to Monitoring Form Reviewer by the 15th of the month
- Help provider get organized with CACFP record keeping while conducting desk reviews in their home/Center
- Inquire of application changes during each visit and amend applications as needed; submit to CACFP Director
- Assist with information in appeals process
- Report any problems that present a threat to the children to the CACFP Director/ CEO immediately
- Report allegations of CACFP child abuse, neglect and non-compliance issues to CACFP Director/ CEO
- Verify recipes & CN labels are on file for required food items that are identified in the menu for the current month. i.e. commercially prepared main dish food items, combination food items, etc...
- Do technical assistance with providers/center as needed; notate topics discussed on the review form
- Review menus for the month while at the providers’ home; provide technical assistance for main dish items that repeat within 10 days. While conducting on site visits, initial the menus.
- Check enrollment forms on all children against the attendance form during the visit (Homes).
- Check the center master roster before going to the center/ before the review to ensure all enrollments are current.
- Verify all enrollment forms are on file at the home/ center
- Verify all infants have an infant statement and an infant feeding plan
- Verify medical statements are on file for children with allergies
- Conduct the 5 day reconciliation before going to the center/ before the desk review
- Check: application, agreement, income eligibility form for providers own children, “Building for the Future” flyer, License, DOH Food Handler License, WIC, Current Month’s Menu, attendance for 12 months during visits, and Justice for All, point of service form (Center), parent sign in sheet, Non Discrimination Statement, Training certificates, Time distribution Report
- Verify letter to households is in the center enrollment packet (centers)
- Complete a summary finding form/corrective action plan if needed during visit with a date to complete the corrective action. Corrective action must be completed prior to the monitor submitting the review form. Revisit the corrective action issue to ensure permanent correction within 2-4 weeks.
- Email, fax, or mail a copy of the review to the provider.
- Submit Payment Request, completed reviews and Projected Schedule to the Monitoring Form Reviewer. Mail by the 15th of the month
New Providers Start Up Procedure
- Complete Pre-Operation Form
Training
- Attend sponsor training and be in attendance at staff meetings/training by way of conference calls when scheduled.
- Attend state training as needed.
Admin
- Collect the Racial and Ethnic data on the first monitoring visit each year. Request the names of the children in attendance and obtain the race and ethnicity from the enrollment/ income form.

Monitors are paid a base pay for each client and provided gas reimbursement.
Job Type: Part-time
Pay: $15.00 - $20.00 per hour
Expected hours: 1 – 5 per week
Schedule:
- Monday to Friday
- Weekends as needed
