JOB DESCRIPTION

JOB SUMMARY
The Intensive Family Support Services (IFSS) Case Manager provides intensive in-home services to families that struggle with meeting the needs of their children and completing their goals. The IFSS case manager also helps lessen the risk of the child and works toward preventing out-of-home placement of the children. The case manager provides ongoing risk assessment to the child, offers family crisis intervention, maintains a caseload of approximately six to eight families, and provides face-to-face contact with the family as mandated by the IFSS program (one to three times a week). The IFSS case manager has a clear and precise awareness of the community and the ability to utilize resources on behalf of the client, network, develop a plan with clients, and advocate for clients and families experiencing duress while maintaining sensitivity to community issues. Provided Transportation for there family as needed. Requires a flexible schedule to accommodate the family’s needs.

Works a Hybrid schedule

EDUCATION
Bachelor of Social Work, Human Services, or a closely related field.

EXPERIENCE
One (1) years or more of experience working in social services, education, health care, or a related field. Experienced in a human services setting, preferably with children, families, and/or minority populations.

Knowledge, Skills, and Abilities

  • Experience and knowledge of developmental stages for all age groups.
  • Ability to mobilize community resources, to help the clients engage in effective problem-solving methods, to guide, to counsel, and to support client and/or family strengths.
  • Ability to assess situations for potential crises, offering support through alternative resolution, utilizing community resources, extended family members, public and private agencies, etc.
  • Ability to develop clear alternatives and to assist in problem solving that empowers the family to take responsibility.
  • Ability to engage resistant populations and build rapport with them.
  • Experience and/or knowledge of how substance abuse, mental health, and/or trauma can impact being an effective parent.
  • The ability to be flexible, creative, and have strong assessment skills is essential.
  • The ability to write clearly, concisely, and professionally is critical.
  • Ability to participate in team meetings, monthly interagency meetings, court, and any other child welfare-related appointments

CERTIFICATIONS, LICENSES, AND REGISTRATIONS

  • Valid driver’s license and the ability to use your own vehicle.
  • Must be free from any communicable or infectious diseases.
  • Clean driving history
  • Provide a valid PA child abuse clearance, an FBI check, and a criminal background check.

Service Delivery

  • Aids the family in their ability to be self-sufficient by teaching such skills as home maintenance, life management, budgeting, and employment.
  • It helps the family identify internal and external stressors that are overwhelming them.
  • Facilitate an increase in the parents’ ability to develop appropriate limits without the use of corporal punishment.
  • Increase the parents’ ability to establish clear roles.
  • Facilitates the development and/or enhancement of decision-making and communication skills in the identified family.

Pay: Up to $21.74 per hour
