We are looking for an experienced Registered Nurse to join our Admissions team at our rehabilitation center. The ideal candidate will have a background in geriatrics and long-term care, and will be responsible for assessing new patients, determining their needs, and providing appropriate care. Responsibilities :
- Provide comprehensive care to geriatric patients.
- Assess patient needs and develop a plan of care.
- Communicate with physicians and other healthcare professionals to ensure patient safety and well-being.
- Provide education to patients and their families regarding long-term care options.
- Maintain accurate documentation of patient information and progress.
- Monitor patient vital signs and administer medications as prescribed.
- Coordinate with other departments to ensure smooth admissions process.
