Key Responsibilities
- Assess patient needs, develop individualized care plans, and provide in-home treatments and interventions
- Monitor patients’ health status and progress, addressing changes in condition and updating care plans as necessary
- Administer medications, wound care, and other treatments while ensuring patient safety and comfort
- Educate patients and their families on managing chronic conditions, medications, diet, and home safety
- Coordinate with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes
- Accurately document assessments, care provided, and patient progress in compliance with home health protocols
