Position Summary:
We are seeking a dedicated Family Nurse Practitioner to join our team and provide high-quality adult medical healthcare services. This full-time position focuses on preventive care, health screenings, comprehensive assessments, and the diagnosis and treatment of acute and chronic conditions.
Responsibilities and Duties:
- Deliver high-quality healthcare by conducting interviews and physical examinations, formulating diagnoses, and instituting treatment plans for adult patients.
- Perform preventive care services, including health screenings and comprehensive assessments, while making referrals to specialty clinics as needed.
- Participate in chart reviews and other activities to ensure high-quality care.
- Collaborate with the medical team to maintain and promote a high standard of care for all patients.
- Enforce and contribute to the policies and procedures manual to ensure compliance with clinic standards.
- Consult with the Assistant Medical Director and Nurse Manager to ensure clinical staff performance aligns with organizational policies and objectives.
- Ensure clinical compliance with Managed Care/HMO, SBHC contract requirements, and federal/state guidelines.
Requirements:
- Current licensure as a Family Nurse Practitioner in the state of New York.
- Minimum of two years of clinical experience as a Family or Adult Medicine Nurse Practitioner.
- Strong communication and collaboration skills, with a focus on delivering patient-centered care.
