Join our client as a Forensic Re-Entry Wellness Coach (BSW) in Brooklyn, NY. This role is designed for a compassionate and resourceful professional to support individuals transitioning from forensic settings into community living. You will assist clients in building independent living skills, securing housing stability, and accessing critical mental health and wellness services. This position is ideal for someone passionate about mental health advocacy, case management, and rehabilitation support.
Compensation: $22/hour
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
Responsibilities as the Forensic Re-Entry Wellness Coach (BSW):
- Client Engagement & Support: Assist individuals with mental illness and re-entry challenges in developing independent living skills and securing supportive services.
- Wellness & Recovery Coaching: Conduct group meetings on wellness management, providing guidance on mental health, substance use recovery, and daily living skills.
- Service Plan Coordination: Complete service plan reviews for 18-20 residents, ensuring alignment with individualized recovery goals.
- Housing Stability & Case Management: Support clients in housing retention, assist in setting rehabilitation goals, and develop personalized Housing Support Plans.
- Apartment & Living Space Oversight: Conduct regular inspections, identify maintenance needs, and ensure safe and stable living conditions.
- Community Resource Navigation: Link clients to mental health services, substance use treatment, and social service programs to enhance recovery and independence.
Qualifications for the Forensic Re-Entry Wellness Coach (BSW):
- Education: Bachelor’s degree in Social Work, Human Services, Psychology, or a related field required.
- Experience: 2 years of experience working with individuals with mental illness, substance use challenges, or a history of homelessness.
- Case Management & Outreach: Experience in scattered-site housing, case management, tenant organization, and community outreach strongly preferred.
- Advocacy & Peer Support: Experience or training in peer counseling, advocacy, or group facilitation is a plus.
- Technical Proficiency: Working knowledge of Microsoft Word, Excel, PowerPoint, Windows Operating System, and email platforms.
- Language Skills: Bilingual English/Spanish preferred.
