Job Overview – Health Homes Care Manager :
Step into a temporary role with our client as a Health Homes Care Manager in Bronx, NY. This in-office position focuses on coordinating comprehensive healthcare services for children and youth, ensuring they receive the necessary medical, mental health, and social support. You will work closely with families, multidisciplinary teams, and community providers to facilitate access to care and improve health outcomes.
Compensation : $25 – $26 / hour
Location : Bronx, NY
Schedule : Monday to Friday, 8 : 00 AM – 5 : 00 PM
Responsibilities as the Health Homes Care Manager :
Enrollment & Documentation : Facilitate intake, obtain consents, and ensure accurate demographic records.
Ongoing Assessments : Conduct CANS-NY assessments every six months and maintain required monthly contacts.
Health Screenings & Coordination : Perform developmental, reproductive health, and HIV screenings per regulations.
Care Coordination : Collaborate with case planners, caregivers, and providers to ensure integrated healthcare services.
Patient Monitoring & Follow-Up : Track condition changes, update care plans, and facilitate treatment adjustments.
Hospital & Emergency Care : Manage notifications and documentation for hospitalizations and emergency visits. Qualifications for the Health Homes Care Manager :
Education : Bachelor’s degree in Health or Human Services, Social Work, Psychology, or a related field required.
Experience : Prior experience in care coordination, case management, or healthcare services.
Technical Skills : Familiarity with electronic health records, New York State CNNX (Connections) System, and other data systems upon training.
Skills & Attributes : Strong communication, time management, and organizational skills with the ability to manage multiple priorities.
Language Proficiency : Bilingual in Spanish / English strongly preferred.
