Job Overview – Care Coordinator
Looking for a fulfilling role in care coordination? Our client is seeking a Care Coordinator to join their program in Staten Island, NY, where you will play a vital role in managing client care and ensuring they receive necessary support and services.
Location: Staten Island, NY
Compensation: $25/hour
Schedule: 9 AM to 5 PM, Monday to Friday
Responsibilities as the Care Coordinator:
- Client Care: Provide coordination for clients, ensuring access to necessary services like medical care, behavioral health services, and social support.
- Advocacy: Advocate for client services via phone and in-person.
- Monitoring: Conduct home visits and follow-up to monitor client status.
- Collaboration: Participate in case conferences and training sessions.
- Networking: Maintain relationships with service providers.
- Documentation: Keep accurate and timely documentation in client charts and records.
Qualifications for the Care Coordinator:
- Education: Bachelor's Degree or High School Diploma with 2 years of care management experience.
- Experience: Experience working with homeless and HIV populations.
- Language Skills: Bilingual skills in Spanish preferred.
- Communication: Exceptional communication skills, both oral and written.
- Licensure: Clean valid driver's license required.
