Schedule: Monday - Friday, 8:00 AM - 5:00 PM
Compensation: $70,000/year
Overview
We are seeking dedicated and qualified health professionals to join our team as Intake Counselors. This role is critical in conducting clinical interviews and assessments to determine client placement within specific program units.
Key Responsibilities
- Conduct comprehensive clinical assessments and reassessments for referred clients.
- Review medical and health-related documentation using standard protocols.
- Complete biopsychosocial assessments using approved techniques and questionnaires.
- Perform evaluations aligned with the Social Security Administration’s disability determination process.
- Refer clients for specialized mental health evaluations as needed.
- Identify functional limitations and determine client placement in one of three units
- Request and review additional assessments to ensure accurate functional capacity determinations.
- Collaborate with physicians, evaluating clinicians, and engagement specialists for follow-up activities and service planning.
- Prepare clinical assessment reports and obtain client signatures for documentation.
- Utilize motivational interviewing techniques to gather comprehensive client information.
- Participate in team meetings and provide input to enhance clinical assessment processes.
- Adhere to policies, procedures, and standard operating protocols.
Qualifications
- Licensure: Must hold a valid license as one of the following: Licensed Social Worker (LMSW or LCSW) OR Licensed Mental Health Counselor (LMHC)
- Minimum of 2 years of clinical assessment experience involving biopsychosocial evaluations, document review, and SSA disability assessment.
- Strong organizational skills and attention to detail.
- Proficiency in motivational interviewing and care management techniques.
