Responsibilities as the Per Diem Licensed Practical Nurse (LPN):
- Patient Care & Monitoring: Assess residents’ physical and emotional well-being, provide wound care, trach care, and diabetic counseling.
- Medication Administration: Ensure accurate medication distribution, educate residents on prescriptions, and maintain electronic MAR records.
- Clinical Documentation: Maintain detailed nursing documentation, including health assessments and treatment records.
- Team Collaboration & Communication: Work closely with nursing staff and program teams, escalating critical health concerns to the Nurse Manager and Program Director.
- Confidentiality & Compliance: Uphold patient privacy, adhere to HIPAA regulations, and maintain a professional and secure care environment.
- Licensure: Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania.
- Education: Completion of an accredited Practical Nursing program.
- Experience: Prior experience in a clinical, long-term care, or rehabilitation setting preferred.
- Technical Skills: Proficiency in electronic medical records (EMR) systems and medication administration procedures.
