What are the responsibilities and job description for the Oral Oncolytics Nurse Navigator - Part Time position at Tahoe Forest Health System?
This position is expected to fulfill 30 hours/week.

Summary
The Oral Oncolytic Nurse Navigator (OONN) manages the complex care processes from the time of diagnosis, through the treatment phase and post recovery phase for each individual patient. This individual is often the first point of contact for the patient once an oral antineoplastic agent(s) is prescribed and ensures care is coordinated. The objective is to streamline the care process, provide ongoing assessment and education and reduce barriers to adherence.
Essential Duties and Responsibilities
- Builds awareness of Oncology Nurse Navigator within TFHD care providers.
- Responds to specialty prescriptions and initiates prior authorization process within 24 hours.
- Demonstrates knowledge and proficiency in the use of the EMR as it pertains to oral oncolytic prescription processing.
- Demonstrates proficiency in patient education and provides information tailored to the individual’s disease process and treatment plan. Assesses the patient’s family health literacy, treatment goals and learning style to ensure effective communication.
- Focuses on the clinical management and care coordination of patients receiving oral oncolytic therapy. Facilitates the care process from insurance authorization/grant funding and medication procurement, patient and caretaker assessment and education, to routine monitoring of side effects, adherence and refill management to ensure optimal outcomes.
- Serves as the primary navigator for patients receiving oral oncolytic therapy in collaboration with the multidisciplinary team including navigation, financial counseling, social work, pharmacy, clinical psychology, dietitian, oncology nursing and medical oncology.
- Assesses for barriers to adherence and identifies appropriate interventions including referrals to specialized services. Documents barriers and coordinates plan of care to address barriers including but not limited to follow-up encounters and early reporting of side effects and symptoms.
- Manages communications from patients, insurance carriers, specialty pharmacies and assistance programs.
- Maintains working knowledge of patient assistance programs.
- Collaborates with financial counselors.
- Collaborates with specialty pharmacies to facilitate procurement of oral oncolytic medications.
- Demonstrates strong critical thinking skills and nursing judgement.
- Manages triage calls from patients and caregiver(s).
- Escalates concerns to the clinical team as appropriate.
- Provides patient and caregiver assessment and education, validated through teach back, including but not limited to:
- instructions for administration.
- use of adherence tools.
- what to do if a dose is missed.
- expected and reportable side effects.
- management of side effects as directed by ordering provider.
- plan for follow-up assessment and education.
- safe handling and disposal of medication.
- Must be able to convey information to patients in a way they can understand and must know when to listen.
- Utilizes translation and language services consistently and appropriately.
- Performs clinical monitoring for side effects and compliance per current ASCO-ONS Standards for Antineoplastic Therapy Administration Safety Standards.
- Follow-up assessment and education may be completed in-person, by phone and/or virtual meeting platform.
- All follow-up encounters will be documented in the EMR.
- Communicates side effects, lab results, triage interventions and coordinates necessary tests to ordering provider.
- Tracks anticipated date of refills.
- Clearly documents the pharmacy utilized for medication(s) ordered in the EMR.
- Organizes and facilitates multidisciplinary care conferences.
- Ability to set priorities and meet deadlines.
- Ability to work independently without close supervision.

Minimum Education/Experience
Associate's Degree Certificate of completion from college or vocational school from a National League of Nursing (NLN), Commission on Collegiate Nursing Education (CCNE) or equivalent accredited school of nursing. The nurse shall have completed a nursing education program that included clinical experiences with actual patients under the supervision of qualified faculty.
3 or more years relevant experience

California Registered Nurse (RN) - Upon hire
Oncology Certified Nurse (OCN) - Within 1 year of hire into job
Basic Life Support (BLS) for Healthcare Providers - Upon hire
BLS: Employee will be enrolled in the Resuscitation Quality Improvement (RQI) Basic Life Support (BLS) Entry or Prep Curriculum (depending on their
previous BLS certification). - Within 3 months of hire into job
Oncology Nursing Society (ONS) Bio-Chemo - Upon hire
Other Experience/Qualifications
Required:
- A minimum of three years experience as an oncology nurse
Preferred:
- One year of Oral Antineoplastic Adherence Navigation
- Oncology nurse navigator experience
- Oncology Certified Nurse (OCN)
- Bachelor's degree (BA, BS, BSN) from four-year college or university
