Summary
The Youth Behavioral Health Care Coordinator is responsible for facilitating healthcare services for patients under 26 years of age, in need of additional education or resources for the management of their chronic, behavioral health or substance use disorder disease. The Care Coordinator works with each patient, and family as appropriate, to develop a patient-centered plan of care specific to their needs and self-management goals. Additionally, the Care Coordinator actively collaborates with the diverse healthcare team and local community partners to improve patient health outcomes.
Essential Duties and Responsibilities
- Through assessment, planning, implementation and evaluation of safe therapeutic care, the care coordinator develops a patient-centered plan of care for medically complex patients.
- Actively collaborates with healthcare team as an advocate.
- Assists patients through the internal and external healthcare systems and community by acting as a patient care coordinator.
- Participates in patient care meetings and multidisciplinary meetings as necessary to meet the patient needs.
- Works with behavioral health practitioners, counselors and intensivist to provide comprehensive approach to behavioral health or substance use disorder treatment.
- Provides patient and family education about common mental health and substance use disorders and the available treatment options.
- Provides brief behavioral interventions using evidence-based techniques such as behavioral activation, motivational interviewing, or other interventions as appropriate to patient and family to help develop coping strategies.
- Supports patient's self-management of disease and behavior modification interventions and assures patient / family knowledge is consistent with care plan.
- Attends clinic visits, field visits and home visits as indicated to monitor patient progress and assess needs.
- Provides clear communication amongst a multidisciplinary care team and treating clinicians by ensuring awareness regarding patient care plan and progress of goals.
- Contributes to the quality initiatives and collection of data associated with the program.
- Participates on a team for data collection, health outcomes reporting, and clinical audits.
- Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
- Demonstrates System Values in performance and behavior.
- Complies with System policies and procedures.
- Other duties may be assigned.

Minimum Education / Experience
Master's Degree in Social Work (MSW) from an educational institution accredited by the Council on Social Work Education
2-3 years relevant experience
Required Licenses / Certifications
Nevada Licensed Social Worker (LSW)
