Job Title: Professional Social Services Worker
Job Summary:
This position serves as the primary discharge planner for individuals who have been hospitalized for an extended period. This role will also require the ability to link individuals to appreciate services and resources in the community, complete assessments and referrals, and work collaboratively from a multi-disciplinary recovery perspective.
Primary Duties & Responsibilities:
- Facilitate groups and/or one-on-one sessions in which you can coordinate, manage, and implement Psychoeducational lesson plans to help individuals deal with difficulties and overcome dependencies
- Provide services to consumers concerning their Active Treatment including but not limited to classes, activities, note writing, counseling and individual therapy
- Provide necessary weekend coverage and support for peers and staff
- Participate in special events and assignments
Minimum Qualifications
Preferred Qualifications:
- Master’s Degree in Social Work, Psychology, Behavioral Sciences
- Current license OR eligibility to obtain licensure to practice as a Licensed Social Worker or Licensed Professional Counselor
