Position Summary: Provide nursing care utilizing individualized patient needs; establishes standards
based on the scope of practical nursing, under the direction/delegation of the RN, NP or MD. Conduct health assessments, provide therapeutic injections, meet with patients for follow-ups.
Job Responsibilities:
- Administer patient care; provide for the basic comfort of patients in accordance to the treatment plan
- Collaborate in sharing information with other members of the medical team; assist in development of solutions to ensure health, comfort and safety of patients served; coordination of patient referrals
- Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patients
- Evaluate and monitor patients using established procedures, policies, guidelines and standards as observed by the RN, NP or MD; take vital signs (temperature, blood pressure, pulse, and respiration)
- Assist patients, family members or other patients with concern and empathy; respect their confidentiality and privacy
- Administer medication accurately; observe patient response and document therapeutic effects; assist medical staff in pharmacy operations by logging medications, audits, documentation and coordination of prescriptions with external pharmacies
- Interpretation and education on treatment plans, test / lab results, medications and diagnoses to patients about their individualized treatment plan; assist in maintaining and coordinating that effective communication among patient, staff and physician; utilize appropriate teaching methods such as one
- on-one and group instruction, demonstration or discussion
- Assist with any behavioral health questionnaires
- Manage adequate inventory of medical supplies and medications
- Execute triage within the scope of licensed practice in an accurate and precise way; obtain patient medical history through interviewing and careful examination
- Communicate punctual change of shift report consisting of pertinent and relevant information for assigned patients
- Maintain safe, secure and healthy work environment by establishing and following standards and procedures; comply with legal regulations
- Manage and maintain patient health records to ensure compliance Participate in required trainings to keep license current with required continuing education
- Other duties as assigned

Job Requirements:
- High School Diploma or GED
- Minimum 2 years’ nursing experience
- Active license as an LPN in Ohio
- Valid driver’s license and current auto insurance
