We are seeking a passionate and detail-oriented Clinical Assessor to join our team in Toledo, Ohio on a full-time, on-site basis.
Position Summary
The Clinical Assessor works under the supervision of the Clinical supervisor and plays a critical role in evaluating and diagnosing individuals seeking mental health and substance use services. Thisposition ensures timely, accurate, and professional assessments to support client-centered treatment planning.
As a full-time, on-site employee, you will work collaboratively with a team of clinicians and administrative staff to provide comprehensive behavioral health evaluations while upholding Talbot Health Services' mission, values, and commitment to high-quality care. This position is for our east toledo location.
Key Responsibilities
Conduct Diagnostic Evaluations (initial and update assessments) to determine appropriate levels of care.
Ensure accurate documentation of assessments, treatment plans, and referrals in accordance with agency policies (within 48 hours).
Maintain “audit-ready” case documentation, including biopsychosocial assessments, diagnostic summaries, progress notes, and discharge plans.
Provide referrals and level-of-care recommendations based on clinical findings.
Collaborate with agency staff to ensure clients receive necessary services promptly.
Stay informed on payer guidelines and insurance paneling to support smooth claims processing.
Utilize electronic health record (EHR) systems efficiently within 60 days of hire.
Maintain strict compliance with HIPAA regulations and federal/state guidelines.
Participate in ongoing professional development (workshops, seminars, and training).
Uphold ethical standards in all clinical practices and maintain active licensure in good standing.
Meet established productivity goals for assessments and clinical services.
Requirements & Qualifications
Required:
Active Ohio LSW or LPC license in good standing.
Bachelor’s degree in a related behavioral health field.
2 years of experience with Ohio behavioral health systems (Ohio Redesign).
Preferred:
Master’s degree in a relevant field.
Higher licensure (LPCC, LISW, LPCC-S, LISW-S).
Certifications: CPR/First Aid/AED, Crisis Prevention/De-escalation training.

Ability to work in various environments, including community-based settings.
Prolonged periods of desk work and computer use.
Ability to lift up to 15 lbs as needed.
