Job Details :
Position - Home Health LPN
Rate - $47 / hr on w2 or $50 / hr on 1099
Contract - 13 weeks
Shift - 8am -4 : 30 pm
Facility - Cooley Dickinson VNA & Hospice (CDV)
Address - 168 Industrial Dr, Northampton, MA, 01060
Schedule :
Monday Friday, 8 : 00 AM 4 : 30 PM
Weekend rotation : 1 in every 4 weekends
Responsibilities :
Provide 6 patient visits per day in collaboration with the care team, which includes RNs, PTs, OTs, and HHAs.
Deliver skilled nursing care under the supervision of the RN Case Manager and / or Clinical Team Manager.
Perform duties in compliance with the State Nurse Practice Act and federal, state, and local regulations.
Administer care in alignment with organizational policies and acceptable standards of practice.
Requirements :
Home care experience is mandatory.
Ability to perform skilled nursing functions as delegated by the RN or Clinical Team Manager.
Must be available for one weekend shift per month as part of the rotation schedule.
Qualifications :
Active LPN license in good standing.
Strong organizational and time-management skills to meet daily patient visit requirements.
Ability to work effectively within a multidisciplinary team.
Compensation :
Competitive pay rates, with W2 and 1099 options available.
Salary : $2 - $50
