Job Title: Social Worker
Facility: Providence ElderPlace - Spokane

Contract: 13 weeks (Extension based on performance and facility needs)
Shift: Monday - Friday, 8:00 AM - 4:30 PM (Weekends not required)
Pay Rate: $54/hr on W2 or $57/hr on 1099
Providence ElderPlace - Spokane is seeking an experienced Social Worker to provide mental health case management for geriatric patients in an adult day care setting. The ideal candidate will have a background in community mental health and experience providing direct care to patients with serious mental health disorders. This role requires strong crisis intervention skills, safety assessment experience, and proficiency in short-term treatment methods.
Responsibilities:- Provid mental health case management and social work services
- Conduct suicide and safety assessments, crisis triage, and mental health evaluations
- Diagnose mental disorders and recommend appropriate treatment plans
- Provide support and intervention for patients dealing with substance use disorders
- Collaborate with interdisciplinary teams to develop and implement patient-centered care plans
- Ensure continuity of care by coordinating with community resources and healthcare providers
- Education: Licensed Social Worker (LICSW preferred but not required)
- Experience: At least 1 year of social work experience working with geriatrics
- Certifications: BLS (Basic Life Support) required
- Skills: Case management, crisis intervention, and mental health assessment
- Licensure: Must have an active Washington state social work license at the time of contract signing (Pending license NOT accepted)
