Job Description :
Delmar Gardens West is seeking a dedicated and compassionate Physical Therapist to join their rehabilitation team. In this role you will assess develop and implement individualized therapy programs to improve mobility relieve pain and prevent or limit permanent physical disabilities in patients. You will work closely with an interdisciplinary team to enhance patients quality of life.
Responsibilities :
- Evaluate patients physical conditions and develop personalized treatment plans
- Implement therapeutic exercises and modalities to improve strength coordination and mobility
- Educate patients and caregivers on exercises and lifestyle modifications for rehabilitation
- Document progress and adjust treatment plans as necessary
- Collaborate with physicians nurses and other healthcare professionals to optimize patient outcomes
- Ensure compliance with state and federal regulations
