Requirements :
Minimum 710 years related experience (MUST have Case Management Managerial / Supervisory / Director experience) Required
BSN or combination of BSN and MSN REQUIRED
The Interim Director will oversee the operations and staff (approximately 24 staff) for Case Management Social Services onsite Spiritual Care services and Utilization Review.
Responsibilities include (but are not limited to) :
Directing the work of the Case Management department in accordance with federal state and local guidelines and regulations
Facilitates appropriate utilization of hospital and community resources to provide high quality costefficient care and achieve throughput length of stay and clinical quality goals
Builds and maintains strong working relationships with post acute partners to ensure successful transitions of care and positive patient outcomes
Reviews readmissions and oversees opportunities to reduce preventable readmissions; presents reports and statistics to the Utilization Review / Quality Oversight Committee
Responsible for the departments budget contracts with external vendors and applicable policies / standard operating procedures / work instructions; plans and implements recruitment and retention strategies for areas of responsibility in collaboration with Human Resources.
