Shift : Monday Friday 8 : 00 AM 4 : 30 PM (Weekends not required)
Pay Rate : $54 / hr on W2 or $57 / hr on 1099
Job Description :
Providence ElderPlace Spokane is seeking an experienced Social Worker to provide mental health case management for geriatric patients in an adult day care setting. The ideal candidate will have a background in community mental health and experience providing direct care to patients with serious mental health disorders. This role requires strong crisis intervention skills safety assessment experience and proficiency in shortterm treatment methods.
Responsibilities :
- Provid mental health case management and social work services
- Conduct suicide and safety assessments crisis triage and mental health evaluations
- Diagnose mental disorders and recommend appropriate treatment plans
- Provide support and intervention for patients dealing with substance use disorders
- Collaborate with interdisciplinary teams to develop and implement patientcentered care plans
- Ensure continuity of care by coordinating with community resources and healthcare providers
