About the Role
As a Client Care Coordinator, you will be the vital link between clients, families, caregivers, and healthcare professionals. This role combines relationship-building, scheduling, quality assurance, and marketing functions to ensure each client receives personalized, high-quality care.
Key Responsibilities

Client Assessment & Matching: Conduct assessments to understand client needs and preferences. Match clients with caregivers based on compatibility, skills, and availability.

Relationship Management: Serve as the primary contact for clients and families, ensuring satisfaction through clear communication, problem-solving, and ongoing support.

Scheduling & Coordination: Manage caregiver schedules, provide coverage during emergencies, and ensure seamless service delivery.

Quality Assurance: Monitor and evaluate care services through regular check-ins and feedback collection to uphold high service standards.

Documentation & Reporting: Maintain accurate records of care plans, client interactions, and communications to support compliance and care continuity.

Community Engagement & Marketing: Develop relationships with referral sources, implement local marketing initiatives, and represent the organization at events and networking opportunities.
