About the MDS Coordinator Role :
As an MDS Coordinator, you will be at the heart of resident care, ensuring accurate assessments and individualized care plans. You'll be responsible for coordinating the Minimum Data Set (MDS) process , crucial for care planning and compliance. Your role directly impacts the quality of care and financial reimbursement, making it essential to both residents' well-being and the facility's success.
Responsibilities : Qualifications :
- Active RN or LPN license in New York State, with a preference for candidates with AANAC certification (Resident Assessment Coordinator Certified).
- Minimum 2-3 years of experience as an MDS Coordinator in a skilled nursing facility, with comprehensive knowledge of MDS 3.0, care planning, and reimbursement processes (PPS, PDPM).
- Strong understanding of federal and state regulations governing MDS and skilled nursing facility operations.
- Exceptional organizational skills, with the ability to manage multiple assessments and meet tight deadlines consistently.
- Proficient in electronic health records (EHR) systems and MDS software; strong analytical skills for data review and quality improvement initiatives.
- Strong interpersonal and communication skills, with a collaborative approach to working with multidisciplinary teams.
- Attention to detail and accuracy, especially in coding and documentation.
