What are the responsibilities and job description for the LVN / LPN - Home Health position at TALENT Software Services?
Primary Responsibilities/Accountabilities:
- Provides coordinates and directs the provision of home nursing care according to physicians orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Home visits. Periodic office meetings. Must have reliable transportation current drivers license and agency-required liability insurance.'
- Average # of patients/day: 5-8
- Points per day/week: How are points broken down 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.
- Guaranteed Hours: No if there is a situation where the contractor is unable to meet their financial needs due to low patient census please reach out to the AM to discuss floating the contractor to a nearby clinic to pick up hours.
- Types of Cases: Start of Care Wound Care Follow up Visits etc.
- Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty
- First-timers accepted: (Y/N) No must have recent specialty experience.
- Weekend REQ: Varies per location and profession please have traveler discuss during the interview.
- Certs REQ: CPR BLS
- Is on-call REQ (Y/N) What is the req Varies per location and profession please have traveler discuss during the interview.
- Territory/County covering: Territory mileage radius Must be willing to travel up to 75 miles from home clinic.
- Pending License accepted: (Y/N) No must have license in hand.
- Some positions specifically in the western territory often have a Baylor Shift schedule: 4x10s Friday-Monday. Most will be standard 5x8s M-F.
- Dress Code: Scrubs
- Charting: All professions must be proficient with HomeCare HomeBase (HCHB) and used it within the last 6 months. If they have used it outside of the last 6 months they may apply but will be required to take a retraining course during compliance. RNs PTs and Ots are required to be proficient in Oasis.
