Registered Nurse - Case Manager

Job Description :

• The RN Case Manager is responsible for Care Coordination, Care Transitions, and Discharge Planning throughout the acute care patient experience.
• Will be responsible for completing timely Initial Assessments, updated notes, and final notes.
• Will complete referrals to post acute levels of care.
• Will coordinate with the multidisciplinary team to ensure the discharge is on track.
