Licensed Clinical Social Worker Needed

Schedule :
- Monday to Friday with rotating weekends
- 8 : 00 AM to 4 : 30 PM

Job Profile Summary :
As a Licensed Clinical Social Worker, you will provide biopsychosocial assessments, crisis intervention, short-term counseling, advocacy, and linkage to resources. You will also plan transitions of care for patients and their families / significant others of all ages in any patient care setting. Your role will include psycho-education and facilitating support groups, serving as a liaison to community programs, and collaborating with the multidisciplinary treatment team on discharge planning. You may also provide education to staff and other hospital departments and participate on committees as requested.
