Social Services/LCSW (Licensed Clinical Social Worker)

We are seeking a dedicated professional to join our team for a full-time position with a 40-hour workweek, Monday through Friday, from 8:00 AM to 5:00 PM. The role requires a Licensed Clinical Social Worker (LCSW) with a valid Maryland State License.

Key Responsibilities

• Provide case management and care coordination in various units including Med Surg, Med Surg Tele, Tele, ICU-CCU, and Observation Units.
• Conduct biopsychosocial assessments, initial evaluations, and develop comprehensive plans of care to meet the needs of patients and caregivers.
• Coordinate services for home care and equipment following discharge, and facilitate referrals to rehabilitation and long-term care facilities.
• Perform discharge and contingency planning, ensuring quality care in a cost-effective manner.
• Engage in long-range planning and decision-making, with a strong emphasis on Medicaid, Medicare, adult and child protective services.
• Document care delivery, patient responses, and ensure documentation reflects understanding of coverage criteria.
  
  

Required Skills And Experience

• Minimum of 2 years of experience as an LCSW, with strong experience in Emergency Department settings.
• Experience in medical-surgical units with a patient ratio experience of 1:18.
• Proficiency in Cerner/Medconnect documentation systems.
  
  

Preferred Skills

• Experience in long-term care and nursing homes.
• Knowledge of benefits eligibility and data abstraction.
• Ability to determine medical necessity per evidence-based guidelines and manage disease.
• Experience in providing crisis intervention and teaching/learning documentation.
  
  

We welcome first-time applicants with relevant and current hospital case management experience on a medical-surgical unit. This position does not require on-call or weekend duties.
