Job Summary:
We are seeking a skilled and compassionate Nurse Practitioner/Physician Assistant to join our healthcare team. In this role, under the direct supervision of a physician, you will provide comprehensive medical care to patients, collaborate with physicians and other healthcare professionals, and contribute to the overall well-being of our patients.
Responsibilities:
- Conduct patient assessments, including medical history reviews and physical examinations
- Perform medical work in the examination, diagnosis and treatment of patients in the Nephrology clinic and chronic dialysis unit.
- Develop and implement patient care plans in collaboration with the healthcare team
- Order, perform, and interpret diagnostic tests and procedures
- Provide patient education on disease prevention, management, and treatment options
- Collaborate with physicians and specialists to coordinate patient care
- Maintain accurate and up-to-date medical records using EMR.
Requirements:
- Valid Nurse Practitioner/Physician Assistant license, DEA licenses in the State of California.
- Master's degree in Nursing from an accredited program.
- CPR with AED certificate is required.
- Minimum of one year of experience working as a Nurse Practitioner/Physician Assistant preferred.
- Bi-lingual (English/Spanish) preferred.
