Requirements : Graduate of accredited school of Pharmacy and current licensure OHPrevious Hospital PharmacyLocals onlyThis role will work in one of our physician practices, Cardiovascular Medicine. The pilot program will consist of calling patients (according to the schedules) to do medication reconciliation over the phone with patients (reviewing current medications and cancelling those they are no longer taking) and then doing medication refills, answering patient questions about medications and working with the physicians as needed for medication related questions / concerns.Schedule : Monday - Friday 8 : 00am - 4 : 30pm EST.
