Job Description
- We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive geriatric care in rehab center settings. In this role, you will develop and oversee personalized health plans, participate in telemedicine visits, and collaborate with interdisciplinary teams to optimize patient outcomes. Utilizing remote patient monitoring tools, you will proactively manage patient health, ensuring early intervention when necessary. Accurate and timely documentation in the Electronic Health Record system is essential to maintain seamless care continuity.
Education & Work Experience Requirements
- Active NP or Compact licensure in Vermont
- Active PA licensure in Vermont
