About the Role
As an LPN/LVN, you will work closely with the Interdisciplinary Team to develop and implement individualized care plans, incorporating nursing diagnoses and interventions. Your responsibilities will also include collecting data on residents' conditions and collaborating with colleagues to ensure seamless patient care.
Key Responsibilities
- Deliver direct and indirect skilled nursing care to residents, following established policies and procedures.
- Collaborate with nursing staff, physicians, and other stakeholders to ensure seamless patient care.
- Develop and implement individualized care plans, incorporating nursing diagnoses and interventions.
Qualifications
- Valid LPN licensure in New York
- BLS certification
- At least 1 year of experience as an LPN/LVN in a LTC facility
- Ability to float between departments and locations after orientation
