Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Care team. In this role, you will deliver high-quality, personalized care to patients in their homes. You will work closely with patients, families, and interdisciplinary teams to promote recovery, health, and independence in a home setting.
Key Responsibilities
Patient Care : Provide comprehensive nursing care to patients in their homes, including assessments, wound care, medication administration, and other treatments as prescribed.
Care Planning : Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and the healthcare team.
Monitoring : Regularly monitor patients' health status, noting changes and responding promptly to complications or emergencies.
Patient Education : Educate patients and families on managing conditions, medication use, and preventative health practices to enhance independence.
Documentation : Maintain accurate, detailed, and timely documentation of care in electronic health records (EHR) in compliance with organizational and regulatory standards.
Coordination : Collaborate with physicians, therapists, social workers, and other team members to ensure holistic and coordinated care.
Regulatory Compliance : Adhere to all policies, procedures, and standards, including infection control and patient safety protocols.
Education :
Graduate of an accredited nursing program (Associate or Bachelor of Science in Nursing preferred).
Licensure :
Current and unrestricted RN license in the state of practice.
Certifications :
Basic Life Support (BLS) required.
Certification in Home Health Nursing (CHPN) is a plus.
Experience :
Previous home care or community nursing experience preferred.
