About the Role
The Physical Therapist provides high-quality, patient-centered care by managing the complete process of therapeutic care for an assigned group of patients in coordination with the physician and the patient care team.
Key Responsibilities
- Evaluate and treat patients within the scope of practice established by law and professional code of ethics.
- Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
- Provide purposeful and skilled therapeutic care at patient's home using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
- Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
- Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.
Qualifications
- Bachelor's Degree in Physical Therapy or related field
- Licensed Physical Therapist
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
