Description: Registered Nurse Case Worker – VA Hospitals

Position Title: Registered Nurse Case Worker

Department: Nursing / Case Management

Location: Various VA Hospitals

Reports To: Nurse Manager, Case Management Director

Position Overview

The Registered Nurse Case Worker at the VA Hospital plays a vital role in coordinating and managing the care of veterans. This position involves conducting assessments, developing care plans, providing case management services, and ensuring veterans receive the appropriate healthcare services. The RN Case Worker works collaboratively with a multidisciplinary team to optimize patient outcomes and ensure that veterans receive compassionate, high-quality care.

Key Responsibilities

  • Patient Assessment: Conduct comprehensive assessments to determine the healthcare needs of veterans, including physical, emotional, and psychosocial aspects of their condition.
  • Care Planning: Develop and implement individualized care plans based on patient needs and preferences. This includes coordinating with various departments, healthcare providers, and services available to veterans within the VA system.
  • Case Management: Serve as the primary point of contact for veterans and their families, facilitating care coordination, advocating for patients, and ensuring timely follow-up. This includes discharge planning, referrals, and addressing any barriers to care.
  • Collaborative Care: Work closely with interdisciplinary teams including physicians, social workers, physical therapists, and other healthcare providers to ensure the seamless delivery of care.
  • Patient Education: Educate patients and their families about their health conditions, treatment plans, and available resources. Empower patients to make informed decisions about their care and provide self-management guidance.
  • Documentation: Maintain accurate and timely documentation of assessments, care plans, interventions, and patient progress. Ensure all patient records are compliant with VA regulations and policies.
  • Advocacy and Support: Act as an advocate for veterans by addressing their healthcare needs, ensuring they have access to necessary services, and helping to navigate the VA healthcare system.
  • Utilization Review: Monitor and evaluate the utilization of resources and services to ensure appropriate care delivery and cost-effective use of healthcare resources.
  • Quality Improvement: Participate in quality improvement activities aimed at enhancing the quality of care and patient outcomes. Assist in the implementation of best practices and evidence-based guidelines.
  • Compliance: Ensure that all activities are compliant with VA policies, federal and state regulations, and professional nursing standards.

Qualifications

  • Education:
    • Bachelor of Science in Nursing (BSN) preferred.
    • Registered Nurse (RN) licensure in the state where the VA Hospital is located.
  • Experience:
    • Minimum of 2-3 years of nursing experience in a clinical setting, preferably with case management, patient advocacy, or working with veterans or military populations.
    • Previous experience in healthcare coordination or care management is a plus.
  • Skills:
    • Strong assessment and critical thinking skills.
    • Excellent interpersonal and communication skills, with the ability to work effectively in a multidisciplinary team.
    • Ability to manage complex cases and prioritize patient needs.
    • Knowledge of VA healthcare services, policies, and resources is preferred.
  • Certifications:
    • Case Management Certification (e.g., CCM) preferred, but not required.
    • BLS (Basic Life Support) certification required.
  • Additional Requirements:
    • Ability to work in a fast-paced, dynamic environment.
    • Ability to handle sensitive and confidential information.
    • Strong organizational and time management skills.
    • Compassionate and empathetic approach to care, with a deep understanding of the needs of veterans.
Work Conditions

  • Full-time position with varying shifts, including weekends and holidays, based on hospital needs.
  • Work may involve exposure to patients with complex medical conditions and varying levels of care needs.
  • Physical demands may include prolonged periods of standing, walking, and lifting.
