Overview
The Medical Social Worker plays a vital role in the healthcare team, providing support and assistance to patients and their families navigating complex medical and emotional challenges. This position involves assessing patients' needs, developing care plans, and connecting them with appropriate resources. The Medical Social Worker will work closely with healthcare providers to ensure comprehensive care, particularly for individuals facing chronic illnesses, mental health issues, or those requiring geriatric support.

Duties

• Conduct comprehensive assessments of patients’ social, emotional, and environmental needs.
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Provide counseling and support to patients and families dealing with mental health challenges, addiction issues, or behavioral health concerns.
• Facilitate communication between patients, families, and medical staff to ensure a cohesive approach to care.
• Maintain accurate medical documentation and records in compliance with healthcare regulations.
• Assist patients in accessing community resources such as financial aid, housing assistance, or rehabilitation services.
• Educate patients and families about medical terminology and treatment options related to their conditions.
• Advocate for patient rights and help navigate the healthcare system effectively.

Skills

• Proficiency in understanding and utilizing medical records and documentation practices.
• Familiarity with medical terminology related to various health conditions, including geriatrics and behavioral health.
• Experience in mental health counseling techniques and addiction counseling strategies.
• Knowledge of ICD-9 coding for accurate documentation of diagnoses.

Pay: $40.00 - $50.00 per hour
