Job Summary
Talin is seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong foundation in medical documentation, a thorough understanding of physiology, and the ability to work independently while managing multiple patient needs.

Duties

• Conduct comprehensive assessments of patients' health status and needs in their home environment.
• Develop and implement individualized care plans based on patient assessments and medical history.
• Administer medications and treatments as prescribed, monitoring patients for any adverse reactions or changes in condition.
• Document all patient interactions, care provided, and changes in health status accurately and timely.
• Educate patients and their families about health management, disease prevention, and treatment options.
• Collaborate with interdisciplinary teams to ensure coordinated care for patients.
• Assist with laboratory tests and procedures when required.
• Maintain compliance with all regulatory requirements and standards of practice.
• Collaborate with interdisciplinary teams to ensure coordinated care for patients.
• Provide support for various specialties including geriatrics, post surgical care, post-stroke care, cardiac care, diabetic care, and renal care.
• Assist with laboratory tests and procedures when required.
• Maintain compliance with all regulatory requirements and standards of practice.

Experience
Candidates should possess the following experience or skills:

• Proficiency in medical documentation practices.
• We use Aloraplus
• Must be knowledgeable documenting OASIS or willing to learn
• Knowledge of physiology relevant to patient care.
• Proficient in wound care a plus.
• Proficient in home dialysis training and management.
• Familiarity with home healthcare a plus.
• Experience working with geriatric populations is a plus.

Pay: $60.00 - $100.00 per hour
