Duties:
- Oversee and coordinate the MDS (Minimum Data Set) process for patients in a healthcare facility
- Ensure accurate and timely completion of MDS assessments and documentation
- Collaborate with interdisciplinary teams to gather necessary information for MDS assessments
- Conduct thorough assessments of patients' medical history, current condition, and care needs
- Develop individualized care plans based on assessment findings and in accordance with regulatory standards
- Monitor and update care plans as needed to reflect changes in patients' conditions or needs
- Coordinate discharge planning and ensure smooth transitions of care for patients
- Maintain accurate and up-to-date health information management records
- Collaborate with healthcare providers to ensure compliance with quality standards, such as HEDIS (Healthcare Effectiveness Data and Information Set)
- Utilize electronic health record systems, such as eClinicalWorks, Cerner, or Athenahealth, to document and track patient information
Requirements:
- Bachelor's degree in nursing or a related field (preferred)
- Valid nursing license (RN or LPN)
- Experience in discharge planning, health information management, or related areas
- Knowledge of regulatory standards and guidelines for MDS assessments and care planning
- Familiarity with pediatric care or experience working with pediatric patients (preferred)
- Strong attention to detail and ability to accurately gather and analyze patient data
- Excellent communication and interpersonal skills to collaborate with interdisciplinary teams
- Proficiency in using electronic health record systems for documentation purposes
